In submitting this Online Membership Application, I certify that the above information is correct and complete. I do hereby agree to follow all the membership rules and protocols of the World Shinmoo Jongshindo Association, World Sinmoo Association and Middle East Shinmoo Hapkido Association: 1. The School owner/member agrees to abide by all the rules and regulations of the World Shinmoo Jongshindo Association, World Sinmoo Association and Middle East Shinmoo Hapkido Association which includes bylaws and all policies. 2. It is the responsibility of the school owner/member to register and renew the membership on an annual basis. 3. Upon registration of the school, it is the responsibility of the school owner to register all individual members within that school. 4. It is the responsibility of the school owner to ensure that all masters, instructors, coaches, referees are registered. 5. It is the responsibility of the school owner to communicate with their own individual members about all the World Shinmoo Jongshindo Association, World Sinmoo Association and Middle East Shinmoo Hapkido Association communications, guidelines, rules and regulations, protocols and other matters. 6. MESHA, WSA, WSJA are only responsible to communicate with MESHA School owners directly. I do hereby declare that I have read and agree to follow all the rules & regulations, protocols and guidelines as outlined above. I certify that the above application has been completed by me. RELEASE of LIABILITY, WAIVER of CLAIMS, AUTHORIZATION of CONSENT of INFORMATION in consideration of the acceptance of the Applicant as a member of the MESHA, WSA, and WSJA and payment of membership dues, the Applicant, his/her heirs, executors, administrators and assigns agree to hold harmless and to waive any claim and to keep indemni¬fied the MESHA, WSA, and WSJA its leadership, directors, officers, members, instructors, officials, employees, agents or representatives from any and all claims, actions or causes of actions, costs and expenses arising out of relating to any activity of the applicant taking part in or being connected to any activity of the MESHA, WSA, and WSJA whether caused by negligence of any of the MESHA, WSA, and WSJA’s directors, officers, members, instructors, officials, employees, agents or representatives. I am also fully aware that in participating in these martial arts organizations there are some inherent risks attached to it which may cause injury, bodily harm, or death. Without listing the generality of the foregoing, the applicant further releases MESHA, WSA, and WSJA and its directors, officers, members, instructors, officials, employees, agents or representatives from any recourse which the applicant may now have or hereafter have resulting from any action or decisions of the MESHA, WSA, and WSJA I consent to provide MESHA, WSA, and WSJA my personal information. The MESHA, WSA, and WSJA will not provide this to any 3rd party outside the MESHA, WSA, and WSJA without consent.
